Consent form for the use of photographs or video for University Project

The Project for the module “Life: A User’s Manual recognises the need to ensure the welfare and safety of all people taking part in any activity associated with our project.
In accordance with our protection policy, we will not permit photographs, video or other images of young people to be taken without the consent of the subject. You will be taking part in Overt Ghost project, the Dehumanizing Effect of Homelessness, where you will be either interviewed, taken your picture of video in order to have solid 5-minute footage for our project. We would like to ask for your consent to take photographs/videos of the event or activity that may contain images of you. It is likely that these images may be used as:

·        a record of the activity of the project
·     in a written evaluation report of the project that will be viewed by our lecturer and the public in order to protest against the misconceptions on homelessness

This project will take all steps to ensure these images are used solely for the purposes they are intended. If you become aware that these images are being used inappropriately you should inform Goldsmiths University immediately.
We would be grateful if you would return this form to professor Bernadette Buckley and can be contacted in b.buckley@gold.ac.uk
